Sepsis survivors face significant health challenges after hospital discharge, particularly when transitioning to skilled nursing facilities (SNFs) for continued care. In response to high 30-day hospital readmission rates among this vulnerable population, Adfinitas Health developed and piloted a Sepsis Recovery Pathway in collaboration with a post-acute facility and the University of Maryland Medical System. The pathway introduced structured clinical protocols, including vital sign monitoring, lab surveillance, antibiotic adherence alerts, increased provider engagement, palliative care assessments, and weekend telehealth visits. These interventions were designed to support early identification of patient decline, standardize post-sepsis care, and strengthen collaboration between SNF staff, medical providers, and hospital teams.
This webinar will share the development, implementation, and outcomes of the Sepsis Recovery Pathway, which led to a 30.4% reduction in 30-day all-cause readmissions during a 10-month pilot. Presenters will highlight how cross-continuum collaboration, evidence-based protocols, and culture change were leveraged to improve care for medically fragile patients recovering from sepsis. Participants will gain practical insights into how similar interventions can be applied in other post-acute care settings to improve quality, reduce avoidable hospitalizations, and support safer transitions for high-risk populations.

Virginia Noftsier received a bachelor’s degree in Business Management in 2000 and began her career in retail administration and purchasing and visual merchandising. She then transitioned to a career in medicine, earning a respiratory therapy degree and a master's degree in physician assistant studies. Most of her clinical practice time has been in the post-acute setting. After 10 years in clinical practice, she now serves as Director of Operations, leading a team of over 65 clinicians in a post-acute medical practice. Her focus is on equipping clinicians to provide the highest quality of care while improving operational efficiency.

Jenn is the Post-Acute Program Manager with UM Baltimore Washington Medical Center (UM BWMC). Through close collaboration and exquisite cat-herding skills, she works with acute, post-acute, and outpatient partners to improve transitions of care and extend evidence-based practice from In-patient to Outpatient settings.
Jenn started her career in public health, gaining experience in community health and health policy at the local and State levels. She came to nursing later in life to add clinical experience and perspective to her public health practice. Jenn gained an interest in transitions of care as a community health nurse with the Anne Arundel County Department of Aging, going into the home of patients with high SDOH needs to help them stabilize in the community after hospital discharge. She brought her interest in care transitions and public health to UM BWMC in 2018.
