Patient Safety Series: BW CARES: From the Hospital to Home and Beyond

This one-hour webinar features the the Circle of Honor award-winning project from the University of Maryland Baltimore Washington Medical Center (UM BWMC). This session will spotlight BW CARES (Community, Access, Resources, Education, and Service), a groundbreaking initiative designed to bridge the gap between hospital-based care and the social realities patients face once they return home.

Participants will learn how the BW CARES Travel Team is transforming post-discharge care by meeting patients where they live. Through home visits and community-based outreach, an interdisciplinary team including nurses, social workers, pharmacists, and community health workers assesses and addresses Social Drivers of Health (SDOH) that influence healing, recovery, and long-term health outcomes.

The webinar will explore how this innovative model improves patient care, reduces readmissions, and redefines what it means to care for the whole person. Key topics will include risk stratification, multidisciplinary collaboration, patient-centered strategies, and the real-world impact of seeing the full picture of a patient’s environment.

Katie Bocchino is an accomplished healthcare leader with more than a decade of experience driving improvements in population health, public health, and community health. She plays a pivotal role in overseeing population health management efforts and addressing social drivers of health (SDOH) within the community. She has been instrumental in creating environments where patients are empowered to manage their health. Katie holds a Master of Science in Health Care Administration (MHA) and is passionate about driving innovative healthcare solutions that improve both individual and community health outcomes. 

Stephanie Bradfield

Clinical Manager, Population Health

Stephanie Bradfield, RN, CCM, brings over 30 years of diverse nursing experience to her work as a dedicated healthcare professional and leader. Her career spans multiple care settings, including home health, inpatient nursing, rehabilitation, and case management. Stephanie’s broad clinical background and passion for patient-centered care have shaped her approach to improving care transitions, supporting vulnerable populations, and advancing collaborative practice. As a Certified Case Manager, she is committed to ensuring quality outcomes and bridging gaps in care delivery.

Cheryl Ruff

Sr. Director of Operations for Primary Care and Population Health

Univ of MD Baltimore Washington Medical Center

Cheryl Ruff is the Senior Director of Operations for Primary Care and Population Health. A native of Maryland, Cheryl has worked for more than 35 years in health care. She has worked extensively with homeless and unhoused populations, and has a strong desire to help patients overcome social barriers that keep them from being well in their community. In addition, she seeks to improve communities themselves by personal outreach. She has experience in federal, state, and faith based organizations, and believes they can work together. Her population health work focuses on access to primary care, and building teams that meet, teach and care for patients no matter where they call home.  Cheryl lives with her family in Annapolis, Maryland.
